This photo was ripped off of p.102 of the book that comes with the "Shine On" box. It is Dave playing a guitar, and I suspect that it was taken aboard his houseboat. There is no mention of it anywhere on the CDs or the packaging, but this is from the first Rose Bowl show. Sound is clear (though not quite as clear as "Live Bell") and consistent throughout; bass is strong and sounds saturated in just a few spots. "Your Favorite Disease" gets a 7 or 7+ on the "inspiration & improvisation" factor. Excellent digital fidelity and stereo separation (DDD), though stereo is a little bit off center, but still great; I'll give it a 9+ or 10 for fidelity and stereo. No close audience noise. Packaging is very low budget, and doesn't even say what city/date it's from, but someone on the Echoes Digest told me it was the first Rose Bowl show, 16 Apr 94. The standard show (no DSoTM set). Also a keeper.
The first of these CDs to appear did indeed lack source info. on the packaging. However, later editions of this same CD correct this by including the source beneath the setlist on the back, albeit in very indistinct script.
